Greetings Hornet Nation athletes, parents and supporters.  Now that Polk County Schools have now announced plans to allow summer athletic conditioning to begin:

Here is a quick guide to make the process of getting cleared and back into action possible.

All student-athletes interested in participating in summer will need the following in order to participate.

  1. Cumulative Overall GPA of 2.o (we will check your GPA after you turn in your paperwork)
  2. Completed and updated (as of June 1, 2020) Physical and Consent Forms (along with the HC Athletic Participation Checklist)
  3. Completion of 3 Mandatory Video/Courses  1) Concussions, 2) Heat Related Illness 3) Sudden Cardiac Arrest

2020-21 Athletic Physical (EL2)  & Consent (EL3) Forms (Please be sure to print/sign and date the highlighted areas)

  • Here is a listing of available medical professionals/facilities that are providing $25 sports physicals for student athletes of Polk County $25 Sports Physicals or you may use your primary physican if you choose.

Mandatory Completion of Video/Courses Before Clearance: 

Here are the links to the 3 online video/courses all student-athletes must view prior in order to be cleared to participate.  Once completing the video/courses, a certificate will be emailed to them and they will just need to forward on to Tyrhon.crawford@polk-fl.net or they can just print out and turn in when they turn in their completed physical. Remember they must complete all three course
